Step 1
File your claim
Fill in insurance claim form(s) as soon as the happening of an event giving rise to a claim and submit to us with the supporting documents.
Step 2
Acknowledge
You will receive an email acknowledge from Liberty that we have received your claim submission.
Step 3
Process your claim
Upon receiving all original documents, Liberty will process your claim.
Step 4
Receive your claim amount
You will receive an email from Liberty that payment will be processed by cheque.
Frequently Asked Questions (FAQs)
- Photographs of the damaged item
- Assessment report from repairer on the cause and extend of damage
- Original invoice/receipt of the loss or damage property
- Quotation for repair/replacement of the loss or damaged item
- Police report if claiming for theft or malicious damage
- Original medical receipts/tax invoices
- Letter of Claim or letter of demand from third party
- Letter from golf club confirming the incident
- Writ of summon received from third party
Please note the insured is not to admit any liability whatsoever to the third party and forward any/all third party correspondence(s) to Liberty.
- Death certificate, autopsy report, coroner’s findings, proof of relationship between insured & claimant (i.e. copy of marriage/birth certificate)
- Medical/specialist report
- Letter from golf club certifying the hole in one by the insured person
- A copy of Hole-in-one certificate
