  • Photographs of the damaged property
  • CCTV footage showing the incident/loss, if available
  • Purchased invoices/receipts of the damaged property
  • Assessment report from repairer on the cause and extent of damage
  • Repair/replacement quotations of the damaged/lost property (please submit the quotations for our approval before making any repairs/replacement)
  • Details of the third party including name, identification number, telephone number and address
  • Name, telephone number and address of any witnesses
  • All relevant writs summons correspondence and documents served on you by any third party
  • Photos of the damaged third party items, if available
  • CCTV footage showing the incident, if available

Please note the Insured should immediately notify claim upon receipt of third party claim or report the circumstances that may give rise to a claim. Insured is not to admit any liability whatsoever to the third party and forward any/all third party correspondence(s) to Liberty Insurance.

  • For death claim:
    • Death certificate, autopsy report, coroner’s findings, proof of the relationship between insured & claimant (i.e. copy of marriage/birth certificate)
  • For permanent disability claim:
    • Medical/specialist report
  • Incident report
  • Police report (burglary, robbery or theft) - must be made within 24 hours of the incident
  • Purchased invoices/receipts of the lost property
  • Accounting records (proof) of money lost and misappropriated
  • Form 2 or Form 2B (send the original to the Labour Department and the copy to us)
  • Original sick leave certificate(s)
  • Original medical expenses receipt(s), if any
  • Insurance Certificate/Policy
  • Commercial Invoice/Packing List
  • Bill of Lading/Contract of Carriage
  • Correspondences exchanged with Carrier and other relevant parties regarding liability for the loss/damage
  • Survey report or other documentary evidence to show the extent of loss/damage
  • Delivery Note/Receipt/Order
  • Repair/Replacement Quotations
  • Claim bill
