Terms and Conditions

Terms and Conditions of Wellness by Libbie Body Check-Up Plan:

1. By utilizing Wellness by Libbie, you hereby acknowledge and agree to be legally bound by the following terms and conditions (“Terms and Conditions”) which shall apply to the booking and health check services (“Services”). Please read the below Terms and Conditions carefully.
2. With the consent of Liberty International Insurance Limited (“Liberty”), MediConCen is entitled to make changes to these Terms and Conditions at any time. Any utilization of the Service will be deemed as acceptance of any such modification.
3. You acknowledge and agree that the health services are provided by health centers engaged by MediConCen. Any health check up will be carried out by the health centers. You acknowledge and agree that Liberty is not associated with or responsible for any health services offered on this Wellness by Libbie and shall not be liable for any fault, negligence and/or default in connection with the health services. You agree to waive any claims against Liberty in relation to the Services.
4. You are required to provide truthful, accurate, up-to-date, and complete information about yourself when making appointments or using the form. It is your responsibility to maintain and promptly update your information to ensure its truthfulness, accuracy, currency, and completeness. If MediConCen has reasonable grounds to suspect that any information is untrue, inaccurate, not up-to-date, or incomplete, MediConCen reserve the right to suspend or terminate your service.
5. You can make appointments for the Service within a range of 3 working days to 3 months from the current date. If the selected Service requires an additional payment, the full payment process must be completed before submitting the appointment. If you fail to complete the entire payment process within the specified timeframe, the appointment will be canceled, and MediConCen shall not be held responsible for any consequences arising from the cancellation.
6. You are required to make a reservation for the Service and complete the payment through our website. If you choose to use a third-party payment service for online transactions, by placing an order, you agree and accept that the collection, processing, and retention of your credit card or relevant information will be undertaken by us and the payment service provider, subject to the terms and conditions of the respective third-party payment service. You agree and acknowledge that you will bear sole responsibility for any losses incurred or arising from your credit card transactions, and under no circumstances shall MediConCen be liable for any total or partial loss.
7. MediConCen reserves the right to refuse or cancel your reservation or use of the service in unreasonable circumstances.
8. The body check-up report does not come with an accompanying explanation. If you require further clarification, please feel free to consult your family doctor for an interpretation of the report's contents.
9. If you fail to collect your report within 2 months after the examination, all reports will be destroyed and will not be kept on record. If you wish to obtain a copy of the report at a later date (e.g. 3 months after the examination), an administrative fee of HKD $450 will be charged. Please note that the copy of the report may not be complete.
10. In the event of any dispute, MediConCen Limited reserves the right to make the final decision.
11. The contents and recommendations in the report are provided for reference purposes only to clients of Liberty. If you have any further questions or doubts regarding the report, please consult your independent doctor for further diagnosis. All information will be kept strictly confidential.
