Frequently Asked Questions (FAQs)

  • Individual medical claims must be lodged within 30 days from date of hospitalisation
  • All medical claims with proof of loss must reach Liberty within 90 days from date of treatment
  • Insured to complete part I of the claim form - Hospitalisation/Surgical
  • Attending physician to complete part II of the claim form - Hospitalisation/Surgical
  • Original official final and itemised medical bill(s) (including but not limited to) hospital invoices, attending physician bills, pre and post-hospital invoices, medical/specialist reports with precise details of the diagnosis and treatment for the sickness/injury
  • For government hospitalisation claim, part II of the claim form - Hospitalisation/Surgical can be exempted if the diagnosis is clearly stated on Medical Certificate or Discharge Summary

No. Liberty does not pay for any medical report(s) obtained from the hospital/clinic/doctor. Proof of illness/injury will be at the expense of the insured/claimant.

No. The documents can be scanned and emailed to us, however, we will need the original copies to process the claim.

Please indicate in the claim form - Hospitalisation/Surgical that you are lodging a claim with another insurance company. Please submit a copy of the settlement advice and invoices in order for us to reimburse the balance (subject to policy terms and conditions). The reimbursement should not exceed the total amount that you have incurred.

This depends on the benefit entitlement in your policy. Please refer to the Schedule of Benefits.

Each admission or surgery must be accompanied by a claim form. The claim form is not required for follow-up visits with the same doctor/hospital.

The portion of costs for which the insured person/claimant is responsible. The deductible/co-insurance is applicable for each and every diagnosed medical condition for which a claim is made within any one policy year.

Please state your request on claim form during submission. A certified true copy of the receipt(s) will be returned for the claims with the remaining balance.

Please issue a cheque payable to “Liberty International Insurance Limited” and mail it to us at Suites 2601-04 & 2607-16, 26/F, 1111 King's Road, Taikoo Shing, Hong Kong, together with the shortfall notice for clearance of outstanding.
