All critical illness claims should be reported within 30 days from date of event to Liberty. Supporting Documents should be submitted to Liberty no later than 90 days from date of event.
Process your claim
Upon receiving all supporting documents, Liberty will process your claim.
Incomplete claim form cannot be accepted for processing of claim.
Further information may be needed.
Receive your claim amount
Reimbursement will be settled by cheuqe to the Policyholder. Notification of payment will be issued for your reference

All critical illness claims should be reported within 30 days from date of event to Liberty. Proof of Loss should be submitted to Liberty no later than 90 days from date of event.
Please attach the following documents together with the claim form.
- Histopathological Report
- Confidential Medical Certificate for your claimed critical illness or performed surgery
- Laboratory, Ultrasonogram, X-Ray and/or MRI Report(s)
- Hospital Discharge Summary / Sick Leave Certificate with Diagnosis
- Patient Card Copy of Consulted Doctor(s)
All medical reports, information and evidences as required by us shall be furnished at Claimant's own expenses.
