You can file your claims by
 

  • Downloading LibertyHK app - scan the QR code available on iOS or Android app store. Login to your Liberty Health Club (LHC) account or register one if you don't have.

Submit claims

Submit claims with all relevant documents within 90 days from the date of consultation.

The screen captured of receipt(s) should show the name of the patient (must be identical with ID card), diagnosis, date of consultation, itemised medical bills with the attending physician’s chop, and signature.

All claims must be lodged with all relevant documents within 90 days from date of consultation.

This depends on the benefit of entitlement in your policy. Please refer to the Schedule of Benefits.

The portion of costs for which the insured person/claimant is responsible. The deductible/co-insurance is applicable for each and every diagnosed medical condition for which a claim is made within any one policy year.

Receipt amount limit at HK$10,000.

All General Physician and Dental claims are eligible, including the following:

  • General Physician
  • Chinese Medical Practitioner
  • Outpatient Specialist Consultation
  • Physiotherapy or Chiropractic Treatment
  • Outpatient Diagnostic Laboratory Test
  • Routine Physical check-up
  • Dental benefit
  • Advance Lab. Test (MRI/CT SCAN/PET SCAN)
  • Day surgery

Once you submit a claim on the app, we will receive your medical claim instantly. Members can submit and track their claims anytime.

It is very user-friendly. After logging into the app, please select “Claim History”. You will be able to check all claims in progress and past claims. You may call our service hotline (852) 2892 3809 or email your claim reference to life.medical@libertymutual.com.hk, we will reply to you.
