Please fill in the claim form (Personal Accident) and mail the original *supporting documents to Liberty at One Raffles Quay #40-01 North Tower Singapore 048583.
*Supporting documents:
- For medical expenses - medical invoices (requires treating doctor to state diagnosis; further information/duly completed medical report may be required if condition is complex)
- For death claim - death certificate, autopsy report, coroner's findings, proof of relationship between insured and claimant (e.g. birth/marriage certificate)
- For permanent disability - medical and/or specialist's report
- For temporary disability - medical leave certificate
Please note the above list of documents is not exhaustive. We will contact you directly if other documents are required.
Acknowledge
You will receive an acknowledgement from Liberty that we have received your claim submission.
Process your claim
Upon receiving all original documents, Liberty will process your claim
Receive your claim amount
You will receive an email from Liberty that payment will be processed to the mode of payment that you have selected.

Please submit to us the claim form (Personal Accident), any bills and receipts you have for claim reporting. All other follow-up bills can be submitted later.
No, copies are not acceptable. We need to have the original medical bills to process the claim.
We will require the medical report when necessary, however, for efficient processing of claim, please submit any medical report which you have in hand with regard to the accident.
No. Liberty does not pay for any medical report(s) obtained from hospital/clinic/doctor. Proof of illness/injury will be at the expense of the insured/claimant.
